Cardboard ballot boxes for membership forms

Cardboard ballot boxes can collect membership forms as well as entries, such as this bright green Club Kambo's box with 'Drop your membership form here!' on the header. The slot sits right below the instruction, and the sides list member benefits like special offers, product releases and competitions. A 'Join today' roundel adds a final prompt.
