Smoked acrylic suggestion box with a clear brochure pocket attached to one side

An office suggestion box with a side pocket keeps blank forms and the posting slot together, such as this smoked acrylic box with a clear brochure holder attached. People take a form, fill it in and post it through the slot on top. The compact size fits on a reception counter or in a staff room.
